Red (header Opscans) and Blue (student response Opscans) are for use with evaluations of classes only. A Red header Opscan must be included on top of each stack of Blue student response Opscans as a header sheet for each class to be evaluated.
*Some forms have space to print questions directly on the sheets (Green and Gray). A master is created either by printing directly on plain paper or by using special templates available from Testing & Data Services. The master is then used to copy the information onto blank forms. The Copy Center is familiar with this process.
Caution: If printing your own questions directly onto Opscans, printing (or copying shadow) must not enter the edges, especially the timing track area (black bars on the left edge), or the solid colored bar on the top. If this occurs, it will delay turnaround time. Also notice orientation differences between the front and back of these Opscans.
There are 10 possible responses for each question on our Opscans; however, we can only accept one response per question.
Response areas for each question are listed 1-10 horizontally, with a number inside each circle to be filled in.
Letters A through J are listed horizontally above response columns on all Opscans except Green and Gray.
To assure forms are processed properly, please observe the following:
Use Number 2 pencils only. Our scanner reads graphite markings. Other numbered pencils and pens may not have enough graphite to be read properly.
Fill in all bubbles completely. Checks, X's, or incomplete circles may not be read by the scanner.
Do not make multiple marks (more than one filled-in bubble per item). The scanner will pick up only one response per item. If questions are of the form "fill in all that apply", etc., they will need to be scored by hand or rewritten prior to distribution.
Do not fold, staple, tape, bend, tear, get wet, or otherwise damage opscans. Remember, these forms must go through a machine. In order to run, the sheets must be flat and in good condition. Forms that are too damaged to run will have to be recopied. However, photocopies will not work, since the scanner must read timing marks at precise locations, among other requirements. (Only the Gray forms are designed to be folded in a specific fashion to envelope size for mailing.)

Gray Opscan (not for use with exams) - Gray and black ink on white paper; 8.5" x 12"; does NOT have areas to write or code in Virginia Tech ID number, Name, Course, Date, Form, Seat Number, or Group, etc. However, this Opscan does have plenty of blank space to the left of the response column on both the front and back. This space can be utilized to print questions directly onto the Opscan. | |

There are spaces to code in Instructor's Last Name, Instructor's ID Number (only if required by department), Course Abbreviation, Course Number, Course Reference Number (CRN/Index), values for any Supplementary Questions, and Instructor Code (if used).
