Test Scoring offers a variety of forms free of charge. The Virginia
Tech print shop can create other forms, but most find the cost
prohibitive.
Some forms have space to print
questions directly on the sheets (Green and Gray). A master is created either by printing
directly on plain paper or by using special templates available from
Test Scoring. Then the master is used to copy the information onto
blank forms. The Copy Center is familiar with this process.
Caution: Printing (or copying shadow) must not
enter the timing track area (black bars on the left edge) or the solid
colored bar on the top.
The following table shows fields available on the
various forms. There are "bubbles" for 10 different responses for each
question (usually marked A-J and 1-10). An X indicates there is room to
write text in the field. An O indicates there is space to mark
"bubbles" so that the response is machine-readable.

*Gray has space to print questions on sheet, three additional items (labeled A-C at bottom) which may be used to identify sheet, and may be folded for mailing. Supplies of this form are limited. Requests for more than 2000 forms may take at least a month to fill.
Test Scoring also offers a special form ("Blue") used for evaluation of classes.
It includes standard questions and space for supplementary questions
and written comments. A "Red" opscan must also be included as a cover
sheet for each class to be evaluated.
Using Forms
To assure forms are processed properly, please observe the following:
Use a No. 2 pencil. Our scanner reads graphite markings. Other numbered pencils and pens may not have enough graphite to be read properly.
Fill in all bubbles completely. Checks, X's, or incomplete circles may not be read by the scanner.
Do not make multiple marks (more
than one filled-in bubble per item). The scanner will pick up only one
response per item. If questions are of the form "fill in all that
apply", etc., they will need to be scored by hand or rewritten prior to
distribution.
Do not fold, bend, tear, or otherwise crinkle the opscans. In
order to run, the sheets must be flat. This includes sheets that may
have been spilled on and dried (coffee, water, etc.). Forms that are
too wrinkled to run will need to be recopied. (Don't use a photocopy
machine!) Note: Gray forms are designed to be folded to envelope size
and may be done so for mailing.
